Ottumwa Home Based Primary Care TRAVEL REQUIRED Travel will be required for home visits. RELOCATION AUTHORIZED No KEY REQUIREMENTS Must pass pre-employment examination, be proficient in written and spoken English, have an advanced Social Workers license, background and-or Security Investigation required and have a Master's Degree in Social Work. DUTIES: The primary duties of the position consist of providing a range of professional social work services and interventions that support veterans assigned to or being assessed for placement in the Home Based Primary Care (HBPC) Program of Geriatrics and Extended Care located at the Ottumwa CBOC, in Ottumwa, Iowa. The goal of the program is to enable veterans who require care to remain in the home rather than in an institutional setting as long as possible and to work with veterans and their families or caregivers to optimize the welfare and quality of life for veterans who require home based care. The incumbent interviews veterans, families, and caregivers. The incumbent uses interviews to establish facts about the veteran's condition, the environment, available support, eligibility, and impact of circumstances on the veteran and recommends or makes referral for HBPC or other appropriate program or directs veteran to appropriate community resources. The incumbent provides case management and employs casework methods and techniques to maintain a therapeutic relationship that will help the veteran work towards the best mechanisms of dealing with his/her condition and environment. The incumbent provides counseling to the patient, caregiver(s), family members, and/or significant others assisting them in coping with the psychosocial stress related to illness and other stressors. The incumbent provides clinical / therapeutic intervention and individual or family counseling. The incumbent implements plans for care and provides follow-up and evaluation of HBPC program care. The incumbent collaborates with interdisciplinary team members to develop treatment plans and involves family and/or caregivers in patient health care planning. The incumbent evaluates veteran's condition and care following referral or placement in the program and maintains comprehensive case documentation. Documentation of community intervention (program overprints, progress notes, social summaries, addendums, consultations, etc.) is performed for each patient admitted to the program. Employee collaborates with team to assure that care is provided which promotes health, safety, welfare and satisfaction of the veteran receiving Home Based Primary Care. The incumbent provides patient and family education regarding the concept of Home Based Primary Care. This is a GS 11 position.
